Media Release Form

Media Releases are used for events which will feature FPCT produced photographs or videos. Media releases are only needed for underage participants. Forms can be emailed to church@fpctyler.com or brought to the Admin office.

Church Event Form

FPCT Church Event forms are used for events which will require the Fellowship Hall, FPCT Grounds or other FPCT Property. Each event held at FPCT will need a separate Event Form filled out at least two weeks in advance. Forms need to be emailed to church@fpctyler.com or brought to the Admin office.
