and adults only. All other churches, groups and individuals must

Complete and submit one form per participant. All fields are required (including the
simple math problem) in order for the form to submit.

Pre-registration closes on Aug. 16, 2012.

After we receive and process your pre-registration form, you'll receive an email
containing your registration forms. The forms are due back in the office by Wednesday,
Sept. 5. Thank you for honoring the deadlines.
